In vivo quantification of blood mixing in single ventricle patients with Fontan circulation using 4D flow MRI

چکیده
Background Single ventricle physiology (SVP) is one of the most severe forms of complex congenital heart disease (CHD). Patients undergo multiple surgical interventions including the Fontan procedure (caval venous return is routed directly to the pulmonary arteries). Despite the apparent success of the procedure, it is unclear why some patients develop ‘failing Fontan physiology’ while others remain asymptomatic. However, there is growing evidence that underlying hemodynamics in the Fontan circulation may

More accurate quantification of pulmonary blood flow by magnetic resonance imaging than by lung perfusion scintigraphy in patients with fontan circulation.
BACKGROUND Quantitative evaluation of pulmonary perfusion using lung perfusion scintigraphy in patients with atriopulmonary anastomosis (APA) or total cavopulmonary connection (TCPC) or partial cavopulmonary connection (PCPC) is difficult because of preferential draining of the venae cavae to one lung. Scintigraphy is the gold standard. Phase-velocity MRI (PV-MRI) is a new technique for determi...
متن کاملCMR-based planning of catheter and surgical interventions for aortopulmonary collaterals before Fontan completion: pilot study of 5 cases
Background Aortopulmonary collateral arteries (APCAs) are frequently observed in single ventricle patients. However those patients require staged surgery toward Fontan completion, APCAs are believed to cause evil situations against Fontan circulation such as impediment of anterograde pulmonary blood flow, volume overload for single ventricle and decrease in systemic blood flow. Therefore, inter...
متن کاملNew insights in the Fontan circulation: 4-dimensional respiratory- and ECG-triggered phase contrast magnetic resonance imaging
Background Evaluation of blood flow characteristics in total cavo-pulmonary connection (TCPC) with CMR remains difficult due to its strong modulation by respiration, and is not yet entirely understood. New approaches using 4D phase contrast magnetic resonance imaging (4D PC MRI) are promising and can contribute to the understanding of hemodynamics in the Fontan Circulation. Our objective was to...
متن کاملQuantification of Caval contribution to flow in the Right and Left Pulmonary Artery of Fontan patients with 4D Flow MRI
Background Knowledge of caval contribution to flow in the Right and Left Pulmonary Artery (RPA and LPA) of Fontan patients is of great interest, because uneven flow distribution to the lungs is associated with pulmonary arteriovenous malformations. However, in Fontan circulation more than 1 source of flow to the RPA and LPA is present, which makes difficult to quantify the flow distribution wit...
متن کاملVentricular afterload and ventricular work in fontan circulation: comparison with normal two-ventricle circulation and single-ventricle circulation with blalock-taussig shunts.
BACKGROUND Recent studies have indicated that there are inherent limitations associated with Fontan physiology. However, there have been no quantitative analyses of the effects of right heart bypass on ventricular afterload, hydraulic power, and resultant overall hemodynamics. Methods and Results- During routine cardiac catheterization, aortic impedance and ventricular hydraulic power were dete...
